ADMISSION REQUIREMENTs:
To obtain services from LARC, the Regional Office that serves the Valdosta and Lowndes County area is the first point of contact. The Region 4 Division of the Georgia Department of Behavioral Health and Developmental Disabilities Office evaluates, coordinates, monitors and contracts all publicly-supported community programs and services. Most of these programs and services are provided using Medicaid waiver funding, though State funded services are sometimes available.
To be considered for Medicaid waiver services, an individual must:
1. Be eligible for Medicaid.
2. Have a diagnosis of mental retardation before the age of 18 or other developmental disability before the age of 22.
3. Live in an institution for people with MR/DD or be at risk of going into such an institution.
Individuals may also contact the Region 4 Intake and Evaluation Office by calling 1-877-683-8557 or 1-229-225-5099.
To apply for Developmental Disabilities Services you must first fill out an application. The application can be downloaded by clicking the link below or you can pick one up at our office. LARC’s role in the Admissions process is to provide you with the information and support needed to apply for services. Please contact Janet Dyess for an application, to schedule an appointment, or if you have any questions.
