COMMUNITY ACCESS GROUP & INDIVIUALS

  • Community Access Services –  State and Medicaid funded - these services are designed to assist the participant in acquiring, retaining, or improving self-help, socialization, and adaptive skills required for active participation and independent functioning outside the participant’s place of residence.  Services can be provided in group or individual settings.

 SUPPORTED EMPLOYMENT & Prevocational

  • Prevocational Services – State and Medicaid funded - these services prepare a participant for paid or unpaid employment and include teaching such concepts as compliance, attendance, task completion, problem solving and safety.  Services can be provided at a facility or through mobile crews/enclaves.
  • Supported Employment Services – State and Medicaid funded - these services offer supports to individuals to obtain and maintain competitive community-based employment at or above the minimum wage.  Services can be provided in a group or individual setting.

ADMISSION REQUIREMENTs:

To obtain services from LARC, the Regional Office that serves the Valdosta and Lowndes County area is the first point of contact.  The Region 4 Division of the Georgia Department of Behavioral Health and Developmental Disabilities Office evaluates, coordinates, monitors and contracts all publicly-supported community programs and services.  Most of these programs and services are provided using Medicaid waiver funding, though State funded services are sometimes available.


To be considered for Medicaid waiver services, an individual must:
1.  Be eligible for Medicaid.
2.  Have a diagnosis of mental retardation before the age of 18 or other developmental disability before the age of 22.
3.  Live in an institution for people with MR/DD or be at risk of going into such an institution.

To apply for Developmental Disabilities Services you must first fill out an application. The application can be downloaded by clicking the link below or you can pick one up at our office. LARC’s role in the Admissions process is to provide you with the information and support needed to apply for services.
